PRIMARY FUNCTION/GENERAL PURPOSE OF POSITION:
Licensed Practical Nurse supports and assists with patient care in a Virginia Healthsource physician practice.
This position requires an LPN degree and a current Commonwealth of Virginia Nursing License.
ESSENTIAL JOB FUNCTIONS:
Obtain vital signs, pulse oximetry, respirations, heights, and weights on all patients at every visit. Assist physician to manage his/her in basket. Able to search for critical issues in-basket and prioritize patient care issue. Alerts physician to any critical lab or test values and follows through on result notification of patients as directed by physician. Manages the day to day activities of the medical home team, over seeing the activities of the Medical Assistant and problem solving with team to keep workflow continuous for the physician. Engages team in daily clinical huddles to plan out interventions that need to be taken with the patient while in the practice. Using the disease specific rooming protocols to guide with interventions with the patients. In addition, communicates with the physician on proactive activities that need to occur with the patient while in the practice. Do basic triage screening of patients calling into practice with acute care needs. Seek assistance from physician or registered nurse with all emergent/urgent calls for final deposition. Draw blood and other laboratory samples, as necessary. Administration of Medication per policy, medication reconciliation, and medication refills Able to schedule appointments in a Athena to facilitate acute patient needs. Able to perform point of care testing according to according to manufacturer and CLIA regulations.
Performs rooming procedures according to policy. Maintains professional behavior when dealing with colleagues, patients, visitors, physicians and supervisors. Is proficient in documentation via the electric medical record and to manage tools within Connect Care. Manages my chart request and incoming patient calls. Manages folders within connect care in –basket. Conduct quality assurance and proficiency testing according to guidelines set forth in the CLIA program (ie. Refrigerator temperature checks, emergency equipment checks, etc.). Able to perform routine care and procedures (hearing test, snellin eye test, irrigation of ear, etc.) Attend annual OSHA training program and comply with OSHA regulations and properly dispose of contaminated materials. Make referrals and obtain authorizations for patients, as required. Discuss normal/abnormal laboratory results and conduct patient education as directed by the physician. Maintains annual competencies, nursing license, and CPR certification. Tracks consults, referrals, labs and test for Medical Home team. Maintain diagnostic equipment according to manufacturers’ and OSHA guidelines. Advise Clinical Supervisor of low supplies, medication inventory and orders appropriately. Perform other duties related to office needs as deemed necessary by the Practice Administrator.
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served. Demonstrates knowledge of principles of growth and development of the life span and possesses the ability to assess data reflective of the patient’s status and interprets the appropriate information to identify each patient’s requirements relative to his/her age specific needs and provides the necessary care as described in departmental policies and procedures.
