Nurse Navigator - West End Internal Medicine

I.Primary Function/General Purpose of Position

The Patient Navigator RN works predominantly within Practice Groups to facilitate care of complicated chronic disease patients with multiple co-morbid conditions. The Patient Navigator RN communicates with physicians, nurses, all departments within the hospital facilities, and community resources to expedite medically appropriate cost-effective care. The Patient Navigator RN applies clinical expertise for the medical management of patients in the community to assess the healthcare need§ to provide community resources and referrals.

II.Employment Qualifications

This position requires an RN degree and a current Commonwealth of Virginia Nursing License. Baccalaureate' degree preferred. At least 2 years nursing experience in community nursing, critical care or emergency department experience preferred. Proficiency in basic computer skills, including the ability to maneuver within a Microsoft Windows environment.
