Services provided throughout Hampton Roads

Position Summary:

The LPN will provide care to hospice patients in accordance with the care plan established by the interdisciplinary team. Work will be performed in a variety of settings and requires travel to provide care.

Essential Functions:

Provides direct nursing care to patients as assigned and within the scope of his or her practice as an LPN.

Provides documentation of care through charting in the electronic medical record and updates care plans as necessary.

Provides care with the focus on patient comfort and, pain and symptom management. Demonstrates strong working knowledge of pain and symptom management.

Within the scope of practice, the LPN will assist with the immediate after death care needs of the patient and family, including calling the funeral home, providing personal care notifying other personnel and complete documentation.

Performs visits, tasks or procedures as assigned by the RN Case Manager and as identified in the plan of care.

Provides education and support to patients and families related to the plan of care, medications, side effects, supplies, medical equipment, the dying process, caregiving responsibilities, symptom management and hospice care and services.

Collaborates with the interdisciplinary team in the provision of care and communicates any changes or updates promptly to the RN Case Manager.

Be available, when possible, to meet patient’s/family’s need for care in times of crisis.

Participates in the hospice Quality Assessment and Performance Improvement Program. Maintains compliance with state and federal regulations and Joint Commission Standards.

Participates in on call, weekend, and holiday rotation as required.

Job Qualifications

Licensure/Certification Required:

Current LPN License in the State of Virginia; current BLS certification. Valid and unrestricted driver’s license with an acceptable driving record

Licensure/Certification Preferred:

Hospice and/ or Palliative certification

Minimum Years and Type of Experience:

One year of nursing experience in medical/surgical, skilled nursing home, hospice or home health

Other Knowledge, Skills and Abilities Required:

Excellent time management and organizational skills. Excellent communication and interpersonal skills. Must have dependable transportation.

Other Knowledge, Skills and Abilities Preferred:

Strong computer skills required, such as email, internet and prior experience working in electronic medical record
