Medical*

Eligible first of the month following one (1) month of employment.

Three (3) Plan Options with different deductible and out-of-pocket maximum levels to choose from:

  • Healthy Choice Gold (Highest employee contribution per pay)
  • Healthy Choice Silver
  • Healthy Choice Bronze (Lowest employee contribution per pay)

A Three-Tier Rx Plan (provided automatically when you enroll in a Medical Plan Option)

  • Generic
  • Brand Name Formulary
  • Brand Name Non-Formulary

Dental*

Eligible first of the month following one (1) month of employment.

  • A 12-month benefit designed to encourage preventive dental care.
  • Plan Year maximum benefit of $2,000 in network and $1,500 out of network
  • Preventive and diagnostic covered at 100% In Network – 100% of R&C, out of network
  • Basic Restorative covered at 90% In Network – 80% of R&C, out of network
  • Major Restorative covered at 60% In Network – 50% of R&C, out of network
  • Orthodontic – Adult and child covered at 60% to a lifetime maximum of $2,000 in network – 50% R&C to a lifetime maximum of $1,500 out of network.

Vision*

Eligible first of the month following one (1) month of employment.

  • Core Plan:
    A 12-month benefits for exam and lenses or contact lenses including the option of a new frame every other year.
  • Buy-up Plan:
    A 12-month benefit provides enhanced coverage for eye exam, frames and lenses or contact lenses, including the option of a new frame every year.

*For each Plan, you can choose from three (3) levels of coverage:

  • Employee
  • Employee + 1
  • Family

Employee contributions will be determined based upon selection of the Plan Options and levels of coverage.

Basic Group Life Insurance and Accidental Death & Dismemberment (AD&D) Insurance

Eligible first of the month following one (1) month of employment.

The basic life and AD&D insurance is equivalent to one (1) time your annual base salary and provided at no cost.

Short Term Disability (STD)

Eligible first of the month following one (1) month of full time employment.

Short Term Disability insurance is provided to you at no cost.  After you have been disabled for 30 days, the STD Plan will cover 60% of base annual salary up to a maximum of $1,800 per week for non-exempt employees.  The Plan will cover 80% base annual salary up to a maximum of $1,800 per week for exempt employees.

Long Term Disability (LTD)

Eligible first of the month following one (1) month of full time employment.

Long Term Disability insurance is provided to you at no cost.  After you have been disabled for 90 days, the LTD Plan will cover 60% of base annual salary up to a maximum of $15,000 per month.

Paid Time Off (PTO)

Eligibility:  Full-time and part-time employees are eligible immediately.  St. Francis grants annual vacation, holiday, and sick time through a Paid Time Off (PTO) program.  Accrual is based on hours paid on a pay period basis not to exceed 2080 hours.  Overtime is not included. Employees may not have a negative PTO balance.  PTO hours are accrued based on the following schedule:

Tuition Assistance Program

Eligible after three months of continuous employment as an employee in good standing.

St. Francis recognizes the importance of life-long education and training. The Company provides maximum of $3,000 for full-time employees and $1,500 for part-time employees in tuition assistance each calendar year for health-related or business-related degree or current job related certification from any accredited college or university.  Minimum grade requirement applies for reimbursement. Pre-approval from your Manager and Human Resources Department required.

Employee Sponsored (For Employees who choose to participate)

Supplemental Life and AD&D Insurance

Eligible first of the month following one (1) month of employment.

You may purchase additional coverage for yourself, spouse or dependent children. 

Voluntary Short Term Disability (VSTD)

Eligible first of the month following one (1) month of full-time employment.

You may purchase VSTD insurance which provides coverage of 60% of your base annual salary up to a maximum of $1,800 per week, for a three week period following a 7 calendar day elimination period.

Health Care and Dependent Care Flexible Spending Accounts

Eligible first of the month following one (1) month of employment.

Pre-tax deductions to reimburse eligible health and/or dependent care out of packet expenses.  You may contribute up to $5,000 each calendar year to both accounts. 

Tax Deferred Annuity (403(b) Plan)

Employee may begin contributing immediately.

Employees have the opportunity to save for retirement by electing to contribute pre-tax dollars to the 403(b) Plan as of date of hire. In addition, employees are eligible to receive employer quarterly match and annual pension contributions after completing one year of service as defined in the Plans (1,000 hours worked) and after attaining age 21.
