Medical/RX Plans

  • Three PPO Options: Healthy Choice Platinum, Healthy Choice Gold, Healthy Choice Silver provided by United Healthcare.
  • Prescription Drug coverage: Two (2) tier High Performance Formulary provided by Express Scripts with enrollment in a Medical Plan.

Dental Plans

Preferred Dentist Program provided by MetLife.

Core Plan and Buy-Up Plan provided by OptumHealth Vision.

Life/AD&D Insurance

Basic Life / AD&D

Employer provided = 1 times annual base salary, minimum $30,000.

Buy Up Life

Employee may purchase coverage = to 1 times (1X), 2 times (2X) or 3 times (3X) salary. Evidence of Insurability required for combined amounts in excess of $750,000.

Dependent Life

Employee may purchase coverage from 3 options (Spouse Guaranteed Issue is $25,000). Evidence of Insurability is required for amounts above $25,000.00.

Buy Up AD&D

Employee / Family option in increments of $25,000 to a maximum of $500,000.

Disability Insurance

Short Term (STD)

Employee / Employer Paid – STD pays 50% of annual salary up to a maximum weekly benefit of $170.00. Employee Contribution – maximum $0.60 per week.

Buy up STD

Employee paid – pays 60% of annual salary up to a maximum weekly benefit of $2,000.

Long Term (LTD)

Employer Paid – LTD pays 50% of annual salary to a maximum monthly benefit of $2,000.

Buy Up LTD

Employee Paid – pays 60% of annual salary up to a maximum monthly benefit of $12,000

Miscellaneous Benefits

Flex Spending

May contribute up to $5,000 annually (Jan – Dec) for reimbursement of eligible Healthcare or Dependent Care expenses.

Time Accruals

  • Vacation Days — Employees accrue vacation time from date of hire; hours based on job title and years of service. Carry over max. 225 hours. Vacation time accrues from hire but you are unable to utilize until you have completed 3 months of active employment.
  • Sick Days — 12 per year – 100 days max. New hires do not accrue sick time during their 90-day probationary period.
  • Holidays — New Year, President's Day, Easter, Memorial Day, July 4th, Labor Day, Thanksgiving, Christmas, Float Holiday. New Hires do not accrue holiday time during their 90-day probationary period.
  • Personal Days — Employees are eligible for a personal day January 1, May 1 and September 1.

Tuition Reimbursement

Must be employed 6 months. Full Time Employees & Part Time Employees are eligible.

403(b) Plan

Eligible for voluntary participation immediately.

Following one year of employment, receive employer match of 50% on the dollar, up to employee 6% contribution (Max 3% match).

Retirement Plan

Employer paid plan and non contributory for the employee, sponsored by the Archdiocese of NY.
