Health Benefits


All regular full- and part-time employees are eligible to enroll in the health insurance plan.  Coverage begins the first day of the month following one month of continuous service.  Newly hired or eligible employees must enroll for coverage within 31 days, or wait until the next annual enrollment period.

Three (3) Plan Options with different deductible and out-of-pocket maximum levels to choose from:

  • Healthy Choice Gold (Highest employee contribution per pay)
  • Healthy Choice Silver
  • Healthy Choice Bronze (Lowest employee contribution per pay)

A Three-Tier Rx Plan (provided automatically when you enroll in a Medical Plan Option)

  • Generic
  • Brand Name Formulary
  • Brand Name Non-Formulary


Eligible first day of the month following one month of continuous service.

  • A 12-month benefit designed to encourage preventive dental care.
  • Plan Year maximum benefit of $2,000 in network and $1,500 out of network
  • Preventive and diagnostic covered at 100%
  • Basic Restorative covered at 90% In Network – 80% of R&C, out of network
  • Major Restorative covered at 60% In Network – 50% of R&C, out of network
  • Orthodontic – Adult and child covered at 60% to a lifetime maximum of $2,000 in network – 50% R&C to a lifetime maximum of $1,500 out of network.


Eligible first day of the month following one month of continuous service.

  • Core Plan:
    A 12-month benefit for exam and lenses or contact lenses.
    A 24-month benefit for frames.
  • Buy-up Plan:
    A 12-month benefit provides enhanced coverage for eye exam, frames and lenses or contact lenses, including the option of a new frame every year.

*For each Plan, you can choose from three (3) levels of coverage:

  • Employee
  • Employee + 1
  • Family

Employee contributions will be determined based upon selection of the Plan Options and levels of coverage.

Basic Group Life Insurance and Accidental Death & Dismemberment (AD&D) Insurance

The basic life and AD&D insurance is provided to all full-time employees at (1) times their annual base salary and to part-time employees, $10,000 in life insurance coverage at no cost to the employee.

Paid Time Off (PTO)

Benefited employees accumulate PTO (combination of vacation, holiday, and personal days) hours based on service beginning the first pay period of employment and can be used after completing a total of 90 days of employment.  PTO hours for part-time employees accumulate according to a rate based on length of service, multiplied by the number of hours paid each pay period (not to exceed 80 hours paid).

Full-time employees accumulate PTO hours based on the length of service:
0-4 years 5.5384 hrs per pay period        0-4 yrs max 184 hours
5-9 years 7.0769 hrs per pay period        5-9 yrs max 244 hours
10-14 years 7.8461 hrs per pay period     10-14 yrs max 274 hours
15+ years 8.6153 hrs per pay period       15+ yrs max 304 hours

Part-time employees accumulate PTO hours based on rate as follows:
0-4 years .0692 hrs per paid hour times hours worked
5-9 years .0885 hrs per paid hour times hours worked
10-14 years .0981 hrs per paid hour times hours worked
15+ years .1076 hrs per paid hour times hours worked

Tuition Assistance Program

Employees must have completed 3 months of benefited full time or 6 months of benefited part time service prior to the established application deadline.

Our Lady of Bellefonte Hospital is committed to a program of professional development which offers tuition assistance to employees seeking an approved health related or business related degree from any accredited college/technical or university.  A grade of "C" or better must be received in undergraduate courses and a "B" or better in graduate courses in order to receive reimbursement.  Tuition includes instructional fees, registration (general fees), and lab fees, but does not include parking or other incidental fees.  The total amount of reimbursement for classes completed during a calendar year may not exceed $3,000 per year for full time employees and $1,500 per year for part time.  Courses must be related to the employee’s present job assignment or directed toward employment in a position that is determined beneficial to the System.  Employees must have completed the “Application for Educational Assistance” form and received Manager and V.P. approval prior to the beginning of class.

Based on critical position needs, as identified by Human Resources, Our Lady of Bellefonte Hospital will pay the total cost of tuition, books and fees at 100%.  If an employee voluntarily or involuntarily terminates employment from the System, or their status changes to less than a full time or part time equivalent there is a two year repayment procedure outlined in the current policy.

Supplemental Life and AD&D Insurance

Eligible first of the month following one (1) month of employment.

You may purchase additional coverage for yourself, spouse or dependent children.

Short Term Disability (STD)

Individual Short Term Disability insurance is available.  The premiums are based on your age when you buy the insurance and will not increase as you get older.  The coverage is voluntary and you decide if you want to apply and how much coverage you would prefer.  You may choose a monthly benefit between $400 and $5000 for a disability resulting from sickness or an off-the-job accident.  Coverage for up to 60% of your gross monthly salary may be offered.

Healthcare and Dependent Care Flexible Spending Accounts

Eligible first day of the month following one month of continuous service. Pre-tax deductions to reimburse eligible health and/or dependent care out of packet expenses.  You may contribute up to $5,000 each calendar year to both accounts.

Tax Deferred Annuity (403(b) Plan)

Employee may begin contributing immediately.

Employees have the opportunity to save for retirement by electing to contribute pre-tax dollars to the 403(b) Plan coinciding with or following date of hire. In addition, employees are eligible to receive employer contributions after completing one year of service as defined in the 403(b) Plan (1,000 hours worked) and after attaining age 21.
